Capillary advancement :

Hair implantation that starts very high and/or in the shape of an M gives a broad forehead and a hard, masculine appearance to the face.

This problem exists on both cis and trans women, and often leads to hairstyles with large bangs if the hair is straight enough.

Hairline lowering or advancement is a surgical treatment that lowers the anterior hairline, resulting in a softer, younger, more feminine face. However, this type of procedure is not indicated for men with a tendency to alopecia, as this will progress despite the operation, and the results will be disappointing.

Lowering is possible between 15 and 25 mm. The degree of advancement depends on the patient's wishes and the elasticity of the scalp. It is also possible to modify the shape of the implant by removing or reducing the gullets.

The best indications are found in women with no tendency to androgenic alopecia and with a large forehead without additional bone requirements (otherwise a complete fronto-orbital plasty with pre-capillary incision should be considered).

Surgical technique :

The incision is made along the anterior hairline, then laterally into the scalp.

A wide detachment is performed behind the scalp and in front of the forehead, followed by skin resection.

The closure is made in two planes with absorbable sutures and in such a way as to encourage hair regrowth through the scar.

Surgery procedure :

  • 24-hour hospital stay (discharge the next day after dressing)
  • 2h intervention
  • Oedema and sometimes bruising for 2 weeks
  • Very mild post-operative pain
  • 4 days of dressing
  • 7 to 10 days of social eviction
  • A few weeks of scalp and forehead sensitivity problems
  • Hair regrowth through the scar can be observed around 2 to 3 months post-operatively. Scarring is usually minimal and can be masked by secondary hair grafts.
  • Scar evolution over 18 months

Operating risks :

  • Infection
  • bleeding
  • Localized hair loss
  • Persistent sensitivity disorders

 

With this procedure, there is an immediate result in a single procedure with minimal or even invisible scarring. The results are very satisfying, with a rejuvenated, more feminine face.
