Facial paralysis is a major disfigurement with aesthetic, but also functional, psychological and social consequences.
Its management in the emergency stage must be rigorous to avoid disastrous ocular consequences. In the sequelae stage, comprehensive facial management by a multidisciplinary team allows for the restoration of static and dynamic symmetry and improves the patients' quality of life.
Several therapeutic approaches can be considered:
- facial physiotherapy
- Botulinum toxin (Botox) injections
- Various surgical interventions are performed depending on the different sequelae: at the orbital level, lengthening the levator palpebrae superioris muscle and reinforcing the lower eyelid with conchal cartilage provide sufficient ocular protection. At the lower level, transposition of the posterior head of the temporalis muscle restores lip symmetry and allows for the rehabilitation of the smile. Finally, the healthy side will be treated with physiotherapy to address its compensatory hypertonia and with botulinum toxin injections.
